*228 only changes the unlock code on NAM1, and - even then - it only works on some phones. If you're still on NAM1, then just try the last four digits of your phone number. *228 shouldn't even work on NAM2, so it honestly sounds as though you're on NAM1.

In that case, you have two other options:

01) If you're really determined to get this on your own, then you can just go through all of the possible default codes one at a time. You know, 0000, 0001, 0002, 0003, 0004, 0005, 0006, 0007, 0008, 0009, 0010, 0011...

02) Alternately, if it's a Verizon Wireless phone (not sure about phones with other carriers), then there's a good chance that you can just stop into a store and have a rep connect your phone to the machine that they use to flash phones, and I'm pretty sure that the rep can use that to view and/or reset the NAM2 lock code.
